There are different techniques of the rhinoplasty chosen by the surgeon depending on the type of the nose structure or the place of the disfigured. Same with the type of the noses, there are also different types of rhinoplasty techniques applying for particular needs. When you visit your doctor for rhinoplasty, they will identify your aesthetic and functional needs and decide the technique.
The first 2 options would be the open or closed rhinoplasty. Closed rhinoplasty, as a less preferred rhinoplasty technique, called also as Endonasal Rhinoplasty. It is performed with the small incisions inside of the nostrils. There is no bridging incision and visible skeleton like in the open rhinoplasty in this type of surgery. The best thing with this operation, there is no visible scar on the nose because of the incision area.
In the open rhinoplasty technique, there is a wide framework make the skeleton visible. It is made by using the trans-columellar incision to connect the right and left nostril incisions. In the open (external) rhinoplasty, there will be scars but the operation area will be more comfortable for the surgeon. It allows a more direct view of the nasal structures for a more predictable result. For that reasons, open rhinoplasty is preferred more by most of the surgeons. Patients can also have only nasal base narrowing surgery. This is usually called as weir excision or weir incision according to the using techniques.
Now we are going to see different types of noses which need different rhinoplasty techniques.
Straighten the crooked nose is one of the most difficult operation types of the rhinoplasty. Because it is a basic to change the position of the nasal bone. For repositioning the nasal bone, the main framework is getting centralized as straight as possible. After the operation, there will be a visible correction with the symmetry of the nasal position.
Irregular Nasal Tip
In order to correct irregular, asymmetric or droopy nasal tip, there are basically two kinds of operation. One of them is to remove the excess cartilage on the nose tip and reshape it. The other one is to divide the depressor septi muscle for fixing nasal tip cartilages to steady support.
Nasal Tip Projection
In order to make the nose accordant with facial proportions, there is a particular operation called Nasal Tip Projection. With this operation, the surgeon decides the measurements to correlate nose size by considering face shape.
To reduce the oversize of the nose, the most suitable technique is open rhinoplasty. The surgery is performed under the general anesthesia and the surgeon reduces cartilages about 1-5 mm, sometimes more. This rhinoplasty type is also aimed to reduce the volume of the nasal tip. Also, it provides a change in the shape and size of the cartilages. When the nose has thick skin, the surgeon can change the technique and operation could be more difficult and longer.
Hooked / Humped nose
For fixing the hooked and humped appearance of the nose, rhinoplasty is the best option. During this particular operation, it is performed a correction of the external nose and nasal septum together. The surgeon removes excess soft tissue and suturing the ligaments that help to transform the nose to the straight position.
The reason of the uneven nostrils could be a deviation of the end of the nasal septum or shape difference in the alar cartilages. Any problem with the septum can also cause a deformation in those cartilages. Some people have uneven nostrils naturally or it can sometimes occur after surgery.
